- 30-minute appointment with Deborah Conrad, PA, who gives you the tools to file the exemption, including your state's specific requirements.
- Optional 15-minute appointment with a GoldCare medical provider to discuss vaccines and the pros and cons of vaccination.
You file the exemption yourself. Every state has different rules and forms.
How to get it
- Log in at app.goldcare.com using Chrome or Brave.
- On Home, click Buy Med-Packs (or New Appointment, then the Med-Packs tab).
- Choose Vaccine Exemption Package and click View details.
- Follow the steps to buy it and book the appointments.
