How do I fill out my intake form?

After you have successfully scheduled your appointment, you will be sent an “Intake Form” to fill out. It will be sent to your messages within GoldCare, and you can access your intake form by either clicking on the link sent in the message, or by clicking on the pop-up notification you receive after booking the appointment.
