Do I need to complete the intake form before my appointment?

We ask that this form be completed right after booking your appointment so that your doctor or clinician can review it in a timely manner, before your appointment. 

Although it is not mandatory to answer all the questions on the intake form, we strongly suggest taking your time and answering as many questions as you can, to the best of your ability, as this will better help your physician provide the best care for you.
